Mohs surgery (also known as Mohs micrographic surgery) is an extremely successful procedure for the treatment of skin cancer. The goal is to remove as much of the cancer as possible; while protecting the healthy tissue that surrounds it. Microscopic layers of skin are cut off one at a time and each layer is examined under a microscope. The procedure is complete once a layer is removed that is free of cancer cells. Mohs surgery is exceptionally successful. So much so that it greatly reduces the chance of needing future treatments or surgery.

Why is it called Mohs?

Some 80 years ago, a doctor named Frederick Mohs developed a treatment for skin cancer. Of course, modern technology has produced other surgical options; however, many surgeons still practice this procedure to treat skin cancer. Mohs surgery is considered the most effective treatment for two of the most common types of skin cancer; basal cell carcinoma and squamous cell carcinoma. It can also be used to treat other kinds of skin cancers.

What to expect

Mohs surgery is typically performed in an operating room, but it can also be operated in a doctor’s office with a nearby lab. The surgery usually lasts about 4 hours and is provided on an outpatient basis. There are cases when the surgery can last longer, so patients who are planning to undergo this procedure should set aside the whole day.

Your dermatologist or nurse will prep the area for surgery, then inject the skin with a local anesthetic so the patient will not feel any pain. The Mohs surgeon will begin removing the visible part of the cancer with a scalpel, removing a thin layer of tissue under the visible cancer, and apply a temporary bandage to the surgical site. Removed tissue layers will be taken to the lab to be examined under a microscope. If there is still evidence of cancer cells, additional layers will be cut off until no more cancer can be seen. Cutting the layers of skin might take only a few minutes, but the analysis of the layers is what can take so much time.
