To Schedule an Appointment for Mohs Surgery
Patients referred to our office by another dermatologist or physician for Mohs surgery or excision may schedule an appointment for both a consultation and surgery on the same day, or schedule a preoperative consultation to discuss the operation, with the actual procedure being performed on another date.
If you have a patient whom you would like to refer for Mohs surgery, we will need a copy of the pathology report (if a skin biopsy has already been performed, which is usually faxed by the referring provider) and your current insurance information.
Mohs Surgery Cannot Be Performed Without a Pathology Report
If you do not have a pathology report, please schedule the patient for an office visit so the lesion of concern can be evaluated. A photo and/or diagram of the skin cancer location for the patient to bring on the day of surgery to confirm the surgical site is extremely helpful but not necessary. Although we do not require a consultation prior to surgery, we would be happy to arrange a patient meeting with Dr. Haberle upon request by you or the patient. There are certain locations where Dr. Haberle might request a consultation prior to scheduling the surgery.
